Information submitted through this website

The appointment request form collects your name, phone number, email address, preferred contact method, patient status, and the type of visit you are requesting. We use that information only to contact you about your request.

Please do not send medical information

This website is not a secure channel for protected health information. Please do not include symptoms, diagnoses, medications, or insurance identifiers in web forms, text messages, or email. Share those details by phone or during your visit.

How we use and share information

We do not sell information submitted through this website, and we do not share it for targeted advertising. Information is used to respond to your request and to schedule or manage your care.

Form submissions are delivered by email to the practice and to the vendor that maintains this website, using a third-party email delivery service. Those providers process the information only to deliver and store the message.

Text messages

Agreeing to text messages is optional and is never a condition of receiving care. If you opt in, messages are limited to scheduling and appointment matters, message and data rates may apply, and you can reply STOP at any time to stop them.

How long we keep website submissions

Appointment request emails are kept only as long as needed to respond to the request and to document scheduling. If you would like a submission removed, call the office and we will handle it.

Website analytics

We may measure how visitors use pages, such as which pages are viewed and which buttons are clicked, so the site can be improved. This measurement records actions only. Form field values, symptoms, and other patient details are never sent to analytics.

You can block or clear cookies and similar storage in your browser settings. The site remains usable if you do.

Children

This website is intended for adults. A parent or guardian should submit requests on behalf of a child. We do not knowingly collect information directly from children through this site.

Your privacy rights as a patient

Your rights regarding your medical records, including access, copies, corrections, and how records may be used and disclosed, are described in the practice's Notice of Privacy Practices, which is available at the office. Call us if you would like a copy.

Changes to this policy

If this policy changes, the updated version is posted on this page with a new date.
