Your first visit sets the tone for everything after it, so we would rather you arrive knowing what to expect.
Before you come in
Send an appointment request through this website or call the office. A request is not a confirmed appointment. A member of our team will contact you to confirm a time and tell you anything you should bring.
What to bring
- Your current glasses
- Your contact lens boxes or your prescription details
- A photo ID
- Your vision or medical insurance card, if you have one
- A list of medications you take
- Any questions you want to be sure you ask
Please share health details in person
Website forms are not a secure place for medical information. Send only the details needed to reach you, and save symptoms, diagnoses, and history for your visit or a phone call.
If you are bringing a family member
Many families schedule visits together. Let us know when you call so our team can plan the timing, and bring current eyewear for each person being seen.
Language
Dr. Pham speaks English, Spanish, and Vietnamese. You are welcome to discuss your eyes in whichever language feels most natural to you.
Forms
There are no downloadable forms on this site yet. Our team will provide any paperwork you need when you arrive, and we will tell you in advance if anything should be completed beforehand.
