Insurance participation and plan networks can change. Acceptance of a plan does not guarantee that a specific service, frame allowance, or lens option is covered. Please verify your individual eligibility and benefits with your insurance provider and with our office before your visit.
Vision Plan or Medical Plan
Two Kinds of Coverage, Two Kinds of Visit
A routine vision exam and a medical eye care visit are often billed to different plans. Telling us why you are coming in helps our team apply the right coverage from the start.
Vision plan coverage
Generally applies to a routine exam and eyewear benefits.
- Routine eye exam, usually on a set schedule
- Frame allowance and lens benefits
- Contact lens allowance, when the plan includes one
- Typically billed when the visit is not driven by a symptom or condition
Medical plan coverage
Generally applies when a visit addresses a symptom, injury, or diagnosed condition.
- Eye pain, redness, discharge, or light sensitivity
- Floaters, flashes, or a sudden change in vision
- An eye injury or foreign-body sensation
- Ongoing evaluation of a diagnosed ocular or retinal condition
These are general distinctions rather than a description of your specific policy. Your plan documents and your insurance provider determine what is covered.

Questions Worth Asking Your Plan
Five minutes on the phone with your carrier usually prevents any surprise at checkout.
- Is a routine vision exam covered, and how often?
- Is medical eye care covered under my medical plan rather than my vision plan?
- What is my copay or deductible for this type of visit?
- Do I have a frame or lens allowance, and when does it renew?
- Do I need a referral or a preauthorization?
- Is MC Eye Center in network under my specific plan, not just my carrier?
